I am in-network with Blue Cross Blue Shield PPO and Blue Choice Plans.

I will verify your benefits prior to starting therapy. I encourage clients to call the number on the back of their insurance card to verify the following information:

What are my in-network benefits to see an outpatient mental health provider?

How much is my deductible and how much of it has been met?

Do I have a co-pay?

Out-of-Network & Self-Pay

I am an out-of-network provider for most other insurance plans. This means that you are responsible for payment, in full, at the time of service.

Please see below to determine whether you may be eligible for out-of-network benefits with your insurance provider.

  • You can typically find your member services phone number on the back of your insurance card.

  • Ask your representative: “Does my plan include out-of-network benefits for mental health care? “

  • Ask your representative: “Do I have a deductible for out-of-network mental health services, and if so, what is the remaining amount I would have to pay before my health plan reimburses for out-of-pocket fees?”

  • Ask the representative: "What is the maximum amount my plan will reimburse for mental health service code 90837?"

    If the representative does not provide a clear answer, ask: "What is the maximum allowed amount for mental health service code 90837, and what percentage of the maximum allowed amount will my plan pay?" (This percentage of the maximum allowed amount is the amount you would receive as reimbursement.)
